AYSA SANCTIONING
This tournament is sanctioned by the AYSA. In order to participate, all players and coaches are required to have a current USSF/USYSA/AYSA or their Affiliates Player/Coach Pass Card. If you are not a member of any of these organizations please call (800) 4-SOCCER to obtain membership information.
SANCTIONING RULES FOR AYSA TEAMS
In order to participate in this tournament all AYSA teams must be in "Good Standing". Please submit for approval a copy of your official AYSA computer-generated roster.
SANCTIONING RULES FOR NON-ARIZONA TEAMS
In order to participate in this tournament all USYS and other USSF affiliates must be in "Good Standing" and obtain Permission to Travel* from their State/National Association. Please send a copy of your completed Approved/Certified Roster along with your Permission to Travel to our office at least 45 days prior to the event.
*Permission to Travel is not required from teams registered with US Club Soccer.
In order to participate in this tournament all teams from outside the United States must have written permission and verification from US Soccer and their country’s Federation indicating authorization to participate in the Blackhawks Invitational. Please send a copy of your completed Approved/Certified Roster (complete with one recent passport size photo of each player and coach), your Permission to Travel and Proof of Insurance to our office at least 45 days prior to the event. International teams (players and coaches) will be required to present passports at registration.

At least one team representative must attend and that representative must present the following documents:
• Copy of a signed medical release for each player.
• Laminated USYSA Player ID Cards with photograph. Recreational level teams providing a certified roster signed by their club registrar are exempt from this requirement. Approval by the Tournament Committee must be made prior to check-in.
• Teams must bring a written copy of the team’s roster. For each player, the roster must include: first and last legal name as shown on the birth certificate, birth date, and jersey number.
• Loan forms, properly signed by the required parties, must be submitted for each loan or guest player, as required by the teams sanctioning organization.
• Travel documents for out-of-state teams as required by their governing association. Typically, only US Club Soccer teams are exempt from this requirement. Teams are encouraged to fax travel documents prior to arriving.
• During the check-in process, the Tournament Committee will inspect all documentation, specifically comparing birthdates and names between documents. After completion of check-in, player passes will be bound (via zip tie) for the duration of the tournament. If, during the course of the tournament, a team’s player passes are found to be unbound, the team may be required to repeat the check-in process before the next match.
No players will be allowed to play without the above documentation.
